As per <br /> Jeff Wong February 25, 1991, all old and historical chemicals have <br /> been manifested and shipped as hazardous waste as of October 16, <br /> 1990. <br /> SCHEDULE OF CORRECTION <br /> Violation #1: Effective immediately, hazardous waste will not be spilled or <br /> discharged onto the ground. <br /> Violation Effective 60 days from receipt of this inspection report, TVG-4 <br /> shall submit a contingency plan to Office of Emergency <br /> Services (OES). (see attachment #2) <br /> Violation #3: Effective 60 days from receipt of this report, a training plan <br /> shall be submitted to PHS-EHD. <br /> Violation #4 Effective immediately, all drums containing hazardous waste <br /> shall be labeled with the following information. <br /> (a) The accumulation start date. <br /> (b) The Words "Hazardous Waste". <br /> (c) Composition and physical state of the waste. <br /> (d) Statement or statements which will call attention to the <br /> particular hazardous properties of the waste. <br /> (e) Your name and address. <br /> When all vio ations have been corrected, please send written certification to this <br /> office at thE letterhead address. This certification shall contain the sample <br /> certification tatement indicated on Attachment I. <br />
