<br />3. Section 6626 .20 (a), Article 2, Chapter 12, Title 22, California Code of Regulations:
<br />Hazardous waste (bottom sludge) was transported for off-site treatment, storage, or
<br />disposal withput a hazardous waste manifest.
<br />4. Section 6626 .23 (a)(4), Article 2, Chapter 12, Title 22, California Code of Regulations:
<br />The generat r did not send manifest # CAD 981385347 38031 to the Department of
<br />Toxic Substa ces Control within 30 days of the shipment.
<br />5. Section 6626 .42 (b), Article 4, Chapter 12, Title 22, California Code of Regulations:
<br />The generator did not send an Exception Report to the Department of Toxic Substances
<br />Control withi 45 days of the date the waste was accepted by the initial transporter.
<br />6. Section 662634 (a), Article 3, Chapter 12, Title 22, California Code of Regulations:
<br />Hazardous w ste (20 yard bin) was stored on site more than 90 days without a permit.
<br />7. Section 6626173, Article 9, Chapter 15, Title 22, California Code of Regulations: The
<br />waste oil filte bin was not closed or maintained to prevent leaks and spills.

<br />Documentation showing that personnel successfully completed an annual review of initial
<br />training was riot available at the time of the inspection.

<br />Personnel training records did not list the job title for each position at the facility related
<br />to hazardous waste management, and the name of the employee filling each position.

<br />11. Section 6626f.16 (d)(3), Article 2, Chapter 15, Title 22, California Code of Regulations:
<br />Personnel tra ning records lacked a written description of the type of introductory and
<br />continuing trz ining.
<br />VIII. OBSERVATIONS:
<br />I arrived at UPRR at approximately 0930 hours and discovered that the entire administrative
<br />staff was being tra sferred or laid off. Mike Brennan, Manager of Administration, recognized
<br />me and explained that the facility would no longer have any administrative personnel to handle
<br />the business. All business operations would be handled out of Los Angeles or Omaha, NE.
<br />Mr. Brennan did n t know who left on site would handle hazardous waste operations. I
<br />informed Mr. Brennan that I was here to conduct a routine inspection of the hazardous waste
<br />operations, and he arranged for me to review the paperwork and tour the facility with Jack
<br />Dishman, the wastewater treatment operator.
