4annu?g <br />'Y <br />Violation #7: Within 15 days of receipt of this report, update perrecords showing that personnel successfully complete <br />review of initial training as related to hazardous waste. If an annual <br />review has not taken place, schedule required training to be <br />conducted within 30 days. <br />Violation #8: Within 15 days of receipt of this report, upgrade personnel training <br />records to include the job title for each position at the facility <br />elated to hazardous waste management, and the name of the <br />mployee filling each position. <br />Violation #9: ithin 15 days of receipt of this report, amend personnel training <br />ecords to include a written job description for each hazardous waste <br />osition. <br />Violation #10: ithin 15 days of receipt of this report, amend personnel training <br />ecords to include a written description of the type of introductory <br />nd continuing training. <br />Violation #11: Effective immediately, all hazardous waste containers shall be kept <br />closed except when adding or emptying waste. All hazardous waste <br />containers shall be maintained to prevent leaks or spills. <br />When all violations have been corrected, please send written certification to this office al <br />the letterhead address. This certification shall contain the sample certification statement <br />indicated on Attachment I. <br />PHS-EHD may conduct a follow-up inspection at the facility to verify compliance. <br />The issuance of this Report of Violation and Schedule of Correction does not preclude <br />PHS-EHD from taking any administrative, civil, or criminal action as a result of the <br />violations noted her in. <br />If you have any ques ions or should require additional information, contact Eleanor <br />Ratliff, Senior R.E. .S., at (209) 468-0334. <br />Ernest M.
