Section 6626116 (c), Article 2, Chapter 15, Title 22, California Code of <br />Regulations: Documentation showing that personnel successfully completed an <br />annual revie of initial training was not available at the time of the inspection. <br />8. Section 6626 .16 (d)(1), Article 2, Chapter 15, Title 22, California Code of <br />Regulations: Personnel training records did not list the job title for each position <br />at the facility related to hazardous waste management, and the name of the <br />employee filling each position. <br />9. Section 6626 .16 (d)(2), Article 2, Chapter 15, Title 22, California Code of <br />Regulations: Personnel training records lacked a written job description for each <br />position relat d to hazardous waste management. <br />10. SCHEDULE OF CORRECTIONS: <br />Violation #1: ithin 30 days of receipt of this report, submit a report to the <br />Department of Toxic Substance Control reporting changes in volume <br />and toxicity achieved through waste reduction. <br />Violation #2: Within 30 days of receipt of this report, submit a source reduction <br />and evaluation review of the facility and prepare a waste <br />inimization plan. <br />Violation #3: ffective immediately, all hazardous waste transported off-site for / <br />reatment, storage, or disposal shall be properly manifested. <br />Violation #4: ffective immediately, manifest copies designated to the <br />epartment of Toxic Substance Control shall be promptly <br />ubmitted. <br />Violation #5: ithin 10 days of receipt of this report, all hazardous waste stored <br />ast 90 days shall be properly disposed of. <br />Violation #6: ithin 10 days of receipt of this report, submit an Exception Report <br />or manifest # CAD 981385347 38031. <br />2 <br />
