Conductor casing installed inside the hollow-stem augers, but not grouted in place. Variations from Work Scope Unusual Field Conditions: None noted Date: December 5, 2008 On-Site Supplier Representative: Allan Dudding Scope of Work Performed: Top 5-foot section of 18-inch auger string removed from the ground by clearing soil around it with airknife. Finish grouting conductor casing to surface. After placing a wood plug on the conductor opening, the surface was patched with concrete pending future well completion. Variations from Work Scope: During grouting, the SJCEHD inspector present on site requests that the augers not be turned during grouting. After partial grouting and removing 15 feet of 18-inch auger, the rig broke down; after exhausting all options, 80 feet of 18-inch augers are left in the hole. Unusual Field Conditions: None noted Date: December 8,2008 On-Site Supplier Representative: Allan Dudding Scope of Work Performed: Drilled MW-8 to 100 feet bgs using 8-inch diameter augers inside the conductor casing,no sample collected. Variations from Work Scope: 10-inch hollow-stem augers were having difficulty turning inside the conductor casings. Driller was concerned that the augers would cause the conductor to spin, disrupting the grout well seal.
