(GTI) on.behalf of the above referenced site. The work <br /> plan outlined procedures for beginning an investigation of the vertical extent of <br /> the petroleum hydrocarbon contamination that was documented at this site <br /> following the removal of three underground storage tanks on March 17, 1999. <br /> In the approval letter PHSIEHD requested submittal of a well permit application, <br /> required $89.00 permit fee and that the fieldwork be scheduled. To date these <br /> items have not been received and the work plan has not been implemented. <br /> Please proceed with submittal of the above requested items and notify PHSIEHD <br /> by January 7, 2000 that the fieldwork has been scheduled.
