Before a NFAR letter can be provided <br /> to you, the on-site monitoring well must be properly destroyed. Note that monitoring wells <br /> in an area of contamination may only be destroyed by removing all of the well contents by <br /> drilling out the original borehole and filling it with grout. Provide a short description of the <br /> method and materials to be used to destroy this well. <br /> Please submit a well destruction permit application, permit/inspection fees of $488, and a <br /> completed Master File Record form to the EHD at your earliest convenience.
