Based on this review, we <br /> have determined $1 , 000 to be reimbursable, however, this amount is <br /> not sufficient for us to issue payment . Requests for reimbursement <br /> must exceed $10 , 000 in eligible costs before payment can be issued. <br /> The disallowed and ineligible costs were determined by our <br /> Technical Review Unit ' s findings . It was determined that the WHF <br /> Environmental Consultants, Inc . , invoice #Mod 0688 was eligible in <br /> the amount of $1, 000 , however, all of the remaining invoices were <br /> for work associated with tank removal and therefore are ineligible . <br /> Additionally, the Banks and Co . invoices #6478 and #7565 were not <br /> for work provided at the 642 No. Hunter Street site, but were for <br /> work at the 6230 Pacific Avenue site . If you have any questions <br /> regarding these findings, please contact Steve Marquez at (916) <br /> 227-0746 . All disallowed costs requiring additional information <br /> must be re-submitted. <br /> We are enclosing a new Reimbursement Request #1 form which should <br /> be filled out, signed and resubmitted with the necessary <br /> documentation in order for this claim to be processed. In order to <br /> expedite your reimbursement request please provide the above <br /> referenced material immediately to this office . We have retained <br /> the documentation you previously submitted with your file .
