REVOCABLE PERMIT <br /> Ground Water Monitoring Well <br /> I <br /> Permit No. : 98-09 <br /> Date: September 18 1998 <br /> Location : 4405 PACIFIC AVENUE <br /> Permittee : ADVANCED GEOENVIRONMENTAL INC . <br /> 4005 N . WILSON WAY <br /> STOCKTON , CA 95205 <br /> In accordance with Ordinance 003-96 , passed by the Council of the City of Stockton on January <br /> 22 , 1996 , a permit revocable at the pleasure of the Public Works Director of the City of <br /> Stockton is hereby granted to : <br /> ADVANCED GEOENVIRONMENTAL INC . <br /> to takeover maintenance two (2) existing groundwater monitoring wells previously installed <br /> under Revocable Permit 97- 19 , issued by the City of Stockton November 7, 1997, and to install <br /> and maintain four (4) additional monitoring wells at 4405 Pacific Avenue , Stockton , CA. <br /> This permit is granted under the following terms and conditions : <br /> 1 . This revocable permit is granted subject to the Standard Conditions Applicable to <br /> Revocable Permits adopted by this City Council by Resolution No . 22 , 578 on December <br /> 51 1960 , and on file in the Office of the City Clerk of the City of Stockton . <br /> 2 . The general location of each well shall be as shown on the map attached . The precise <br /> location shall be determined in the field by the City of Stockton . <br /> 3 , The Permittee has posted security of $ 3 , 000 . 00 with the City of Stockton which will <br /> remain in effect during the life of this permit. <br /> 4 . In accordance with Section 1 (h ) of the "Standard Conditions Applicable to Revocable <br /> Permits , " the permittee shall provide and keep in force during the term of the permit a <br /> policy of public liability insurance against liability for injuries to persons or property <br /> arising out of accidents attributable to the structure on City property, in amounts to be <br /> determined by the Risk Manager. The policy of insurance so provided shall contain a <br /> contractual liability endorsement covering the liability assumed by the Permittee by the <br /> terms of his permit and shall contain a provision that such policy may not be canceled <br /> except after thirty (30) days notice in writing to be given to the Risk Manager.
