I ENVIRONMENTAL HEALTH <br /> 4415 Pacific Avenue, Stockton, California PERMIT/SERVICES <br /> IF IIF <br /> Advanced GeoEnvironmental, Inc. (AGE) is hereby granted permission to perform a subsurface <br /> investigation at 4115 Pacific Avenue in Stockton, California. The subsurface investigation is being <br /> performed at the request of Pacific Car Wash. <br /> AGE will perform all necessary work during normal business hours. The scope of work includes the <br /> advancement of a minimum three (3) soil probe borings utilizing a Geoprobe 5400 probing unit. The <br /> scope of work should be completed in one ( 1 ) day. The property owner will be notified at least 48 - <br /> hours prior to performance of field operations. <br /> All costs for performance of the subsurface investigation will be the sole responsibility of the property <br /> owners at 4405 Pacific Avenue (Pacific Car Wash). The owners of 4405 Pacific Avenue will be solely <br /> responsible for all costs and expenses for investigation and/or remediation of contamination at 4415 <br /> Pacific Avenue which is associated with contamination at 4405 Pacific Avenue. <br /> Upon completion of the investigation, the area(s) investigated will be returned to conditions prior to <br /> performance of the subsurface investigation. - <br /> The property owners of 4415 Pacific Avenue will be provided with copies of all testing and reporting <br /> for any investigation performed at 4415 Pacific Avenue.
