Hunter <br /> Stockton, CA 95203 <br /> Re: Information Necessary to Properly Destroy Site Monitor Well MW-8 and Install <br /> Required Monitor Well MW43, Canapa's Car Wash, 6230 Pacific Avenue, Stockton, San <br /> Joaquin County, California <br /> Dear Mr. Canepa: <br /> This letter is in regards to the installation of a new monitor well to be located in the City of <br /> Stockton Right-of-Way on the east side of Evelyn Avenue. Mr. Jeffrey Wong of the San Joaquin <br /> County Environmental Health Department (SJCEHD) indicated in a letter dated February 4, <br /> 2003, that he is requiring the installation of an additional off-site, down-gradient monitor well <br /> (proposed MW- 13) as a condition to approving the proper destruction of site monitor well MW-8 <br /> (necessary for the proposed property developmentT. Mr. Wong specifically indicated that the <br /> submittal of a work plan describing the monitor well destruction and installation was necessary. <br /> Condor subsequently prepared and submitted the Work Plan for Monitor Well Closure and <br /> Monitor Well Installation, dated February 5, 2003 . <br /> To properly destroy site monitor well MW-S in a time frame that meets your construction needs, <br /> Condor respectfully requests your timely consideration of the following requirements. The City <br /> of Stockton requires a Revocable Permit (Application, Attachment A) and an Encroachment <br /> Permit for the drilling of a monitor well within their right-of--way (Evelyn Street). Please find <br /> attached to this letter the necessary information to obtain these two permits for the installation of <br /> the required monitor well MW- 13 , satisfying the requirements of the San Joaquin County <br /> Environmental Health Department (SJCEHD). Without these permits, Condor cannot arrange to <br /> have MW-8 destroyed and replaced, in a down-gradient location, with MW-13, per Mr. Wong' s <br /> requirements. <br /> Information, Signatures, and Fees Necessary for the Revocable Permit <br /> The following actions are required: <br /> 1 . Contact your insurer to obtain a Certificate of Insurance for the Canepa Car Wash Pacific <br /> Avenue Site that will include the City of Stockton as an Additional Insured for at least <br /> $ 1 ,000,000 .00 General Liability. Attachment B to this letter is an example of the <br /> necessary requirement. This insurance will need to be kept in force for as long as the new <br /> monitor well exists. <br /> 2. Contact your insurer regarding obtainment of a Bond of $500 .00 that is also required to <br /> be submitted to the City of Stockton Public Works Department for the installation of <br /> ENVIRONMENTAL GEOLOGICAL 0 ENGINEERING GEOTECHNICAL <br /> http://www-condorearth.com <br />
