(Repeat) An emergen y coordinator and modified contingency plan information is lacking. <br /> -- There must be at least ne emergency coordinator on site or on call to coordinate emergency <br /> response measures, and t e following information must be posted by a phone: the name and phone <br /> number of the emergency oordinator; location of fire extinguishers, spill control equipment, and if <br /> present, fire alarm; and th phone number of the fire department, unless the facility has a direct alarm. <br /> * Immediately appoint an mergency coordinator and post the required information by a phone. Violation was corrected on site. <br /> NOTES (The following are not violations) <br /> The facility generated 840 pounds of hazardous waste in 2010. Facility is a business that regularly <br /> generates used oil, waste nti-freeze, and hazardous waste related to auto repair industry. <br /> No Universal Waste was o served during this inspection. Generator claimed no Universal Waste had <br /> been generated in recent y ars. Discussed with generator Universal Waste management requirements <br /> including aerosol cans, la ps, alkaline batteries, mercury containing switches, CRT, electronic devices, <br /> etc. in a closed container ith a label identifying the content and the start date of accumulation. <br /> Universal Wastes must be nanaged within 1 year of accumulation, and the disposal records maintained <br /> on site for at least 3 years. <br /> A copy of Auto Repair Best Management Practice factsheet, CUPA class schedule, blank hazardous <br /> waste labels, lead acid batt ry management factsheet, and San Joaquin County Small Business <br /> Hazardous Waste Disposal Program pamphlet, have been provided to generator during this inspection. <br /> Submit the following to EH by: July 6, 2011 <br /> 1) A completed copy of he (provided) Return to Compliance (please keep original for your records). <br /> 2) A written statement h w each violation was corrected <br /> 3) Evidence (i.e. photos disposal receipts, documents) in support of the corrections made. <br /> ALL EHD STAFF TIME ASSOCIATED WTH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THISFACILITY 1S,,,SUBgCT TO REINS PECTIO AT NY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD n pecto R Title: <br /> EH 23-02-003 1a08110/10 CONTINUATION FORM <br />
