MONITORING WELL ABANDONMENT ACTIVITIES <br /> A permit for replacement of original monitoring well MW-13 was obtained from the SJCEHD A <br /> Copy of permit is included with Appendix A A copy of the driller's field log is included with <br /> Appendix B <br /> Drilling activities for abandonment of well MW-13 was performed by Cascade Drilling Inc under <br /> the supervision of the SJCEHD Well abandonment activities were performed utilizing a CME-75 <br /> mobile drill ng equipped with 10'/4-inch hollow stem augers,original well MW-13 was over-drilled <br /> to a total depth of 100 feet bsg The well casing from MW-13 was removed from the boring and <br />' • inspected by a representative of SJCEHD, following inspection, the above referenced well casing <br /> was hauled off-site for landfill disposal <br /> The over-drilled bonng was backfilled with a portland type H cement grout mixture Twenty-four <br /> 94-pound bags of portland cement were utilized to backfill the boring, a ratio of six gallons water <br /> for every 94-pound sack of portland cement was utilized in preparation of the grout mixture The <br /> boring was completely backfilled to surface grade with the portland cement grout mixture utilizing <br /> the string of hollow stem auger flights as a tremae pipe <br /> 3.0. MULTI-LEVEL MONITORING WELL INSTALLATION <br /> On 24 October 2002,a replacement well, multi-level ground water monitoring well MW-13 (A-C), <br /> was installed at the site a few feet adjacent the former location of MW-13 <br /> 31 DRILLING <br /> A soil boring was advanced for MW-13 (A-C)vertically to a depth of 152 feet bsg utilizing a CME- <br /> 75 high torque,truck-mounted drill ng equipped with S'/4-inch diameter hollow-stem augers All soil <br /> I cuttings generated from drilling activities were stockpiled at the site and covered with plastic <br /> sheeting in a east section of the site, the location of the soil bonng/monitoring well is depicted in <br /> Figure 2 Ground water monitoring well installation procedures for the site were outlined in AGE's <br /> IAdvanced GeoEnvironmental,Inc <br /> 1 <br />
