INTRODUCTION <br /> In accordance with the request of Mr Art Wyatt, Advanced GeoEnvironmental, Inc (AGE) has <br /> prepared the enclosed Work Plan Addendum for the site at 4511 Pacific Avenue in Stockton, <br /> California(the site) The scope of work includes the installation of four ground water monitoring <br /> wells and the collection of soil and groundwater samples The location of the site is illustrated on <br /> Figure 1 - Location Map A plan of the site is illustrated on Figure 2 - Site Plan <br /> Background information and a description of previous investigations at the site is included in <br /> numerous quarterly reports prepared by Geological Audit Services, Inc (GAS) and AGE A <br /> Corrective Action Plan, recommending soil remediation, was also prepared by Upgradient <br /> Environmental <br /> This work plan was prepared in accordance with San Joaquin County Public Health Services - <br /> Environmental Health Division (PHS-EMD) and Central Valley Regional Water Quality Control <br /> . Board guidelines for investigation of underground storage tank (UST) sates <br /> 2.0. SCOPE OF WORK <br /> 21 DRILLING AND SAMPLING <br /> The scope work to be performed was agreed on during a meeting between the property owner, AGE <br /> and the San Joaquin County Public Health Services - Environmental Health Division (PHS-EHD), <br /> held on 08 September 1996 The PHS-EHD requested that the property owner assess the lateral <br /> extent of the dissolved hydrocarbons on-site and, if necessary, off-sate The PHS-EHD also <br /> requested a receptor (well) survey be conducted for the local vicinity and a reporting of all recent <br /> dispenser repairs conducted at the site <br /> Based upon the scope of work determined during the 08 September 1996 meeting with the PHS- <br /> Er-In n n i~ pVpoJLJ + A » (la +_11 r~ rr,—.—A _+ —+ v`velii <br /> &Lw LM, liu iiOcia AviirVU14 wl livriikV1JL116i <br /> � <br /> • One monitoring well will be located in the northeast corner of the site <br /> • One monitoring well located at the northwest corner of the station building <br /> • One monitoring well located in the parking lot to the west of the site <br /> • One monitoring well located in the southeast portion of the site <br />
