Violation # 209: <br /> Comments noted and discussed with designated operator to ensure compliance. <br /> 4). Violation #211: <br /> The employee training long was corrected on site to reflect the hire dates of the employees and <br /> the corrected copy is attached for reference. <br /> 5).
