The <br /> missing alarms include: Q1 PLLD gross line fail on 10/15/2017.Also the designated operator did not print out the <br /> prove alarms nor did he print out the Electronic line leak detector(Q1)alarms in the last 12 month reports. In their <br /> monitoring plan this facility list audible and visual alarms as overfill preventions. During the monthly inspection,the <br /> designated operator shall review the alarm history for the previous month, check that each alarm was documented <br /> and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> This is a repeat violation, Class ll. <br /> 211 CCR 2715(0(2) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Maria Pedroza last trained on 10/6/2016, Bhadar Singh and Gurman <br /> Singh were last trained 5/8/2017 and therefore not current. The designated operator shall train facility employees <br /> for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 <br /> months. The training shall include,but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site,and submit a copy of <br /> the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 301 HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> Fuel filters were observed outside of all under dispenser containments at the facility.The underground storage tank <br /> system shall be operated to prevent unauthorized releases. Immediately contact a properly licensed,trained, and <br /> certified contractor to replace or upgrade the component under permit and inspection with the EHD <br /> This is a Class II violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 91 product and 87-2 product fill sump. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician <br /> removed all of liquid from the fill sumps.
