Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility by April 22, 2013. <br /> 105. UST facility and tank forms on file with the EHD are not current. Any change of information on <br /> these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and <br /> submit a copy of the facility and tank forms to the EHD. Complete a separate tank form for each tank. <br /> 106 & 202. Current approved copies of the monitoring and response plans were not found on site. The <br /> monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br /> of the monitoring and response plans for approval by the EHD by April 22, 2013. An approved copy will <br /> be returned for your records. <br /> 113. A new designated operator was hired on May 31, 2012, and notification was not provided to the <br /> EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 days. Submit <br /> notification to the EHD identifying all the designated operators for this facility by April 22, 2013. <br /> 117. The Veeder Root TLS-350 was cold started on January 1, 2013 and a permit was not obtained from <br /> the EHD. Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed, trained, and certified contractor pursuant to the EHD's policy and a <br /> permit obtained within one business day from the EHD. All other repairs must have a permit submitted <br /> to and approved by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the <br /> cold start of the Veeder Root TLS-350. A "Failure to Obtain a Permit" penalty applies. <br /> 204. The designated operator failed to check and note the correct test dates for the secondary <br /> containment test on the May 2012 through February 2013 designated operator monthly inspection <br /> reports. The test date was left blank. The most recent secondary containment testing occurred on March <br /> 16, 2011.
