Yosemite Ave. Manteca CA 95336 Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I, CLASS II,or MINOR-Notice to Comply) <br /> and maintenance for the UST system have been completed and document the dates they were done. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. <br /> 205. The March and April 2012 designated operator monthly inspection reports were not found on site. <br /> Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br /> Locate and ensure that copies of the previous twelve months of designated operator monthly inspection <br /> reports are maintained on site. Submit copies to the EHD by April 22, 2013. <br /> 206. The designated operator employee training documentation for Sunny Bhullar, Virginia Durao, and <br /> Sabi Singh was found on site to prove that they were properly trained within the last twelve months. The <br /> designated operator shall train facility employees for which he or she is responsible in the proper <br /> operation and maintenance of the UST system once every 12 months. The training shall include, but is <br /> not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and <br /> submit a copy of the training records to the EHD by April 22, 2013. <br /> 303. The 91 and 89 fill sump sensors and the 87, 89, and 91 turbine sump sensors were touching the <br /> bottom of the sumps, but approximately two inches away from the lowest point of the sumps and not <br /> properly located to detect a leak at the earliest opportunity. The 87 fill sump sensor was touching the <br /> bottom of the sump, but approximately five inches away from the lowest point of the sump and not <br /> properly located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to <br /> be able to detect a leak at the earliest possible opportunity. During the inspection, the technician <br /> repositioned the sump sensors to the lowest point of the sumps and located them to detect a leak at the <br /> earliest opportunity. Continue to ensure that all sensors are properly located at the lowest point of the <br /> sumps to detect a leak at the earliest opportunity. <br /> 305. The 89 fill sump sensor failed to activate an audible and visual alarm when tested to ensure <br /> continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an <br /> audible and visual alarm or stop the flow of product at the dispenser when it detects a leak.
