The missing alarms include a Q1 87 leak detector that had a <br /> gross line fail alarm that occurred on 9/19/2016. These records shall be maintained on site for at least three years. <br /> Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and <br /> maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the <br /> field; (5)logs of all readings of gauges or other monitoring equipment,ground water elevations, or other test results; <br /> (6)results of inventory readings and reconciliations. Immediately locate and maintain all missing maintenance and <br /> monitoring records for the last three years on site and submit copies along with documentation of the alarm follow <br /> up to the EHD. <br /> This is a minor violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator(DO)failed to document all the alarms from the attached alarm history on the designated <br /> operator monthly inspection reports and failed to check that they were responded to appropriately. <br /> On 10/6/2016,the DO failed to document a 87 regular PLLD gross line fail alarm on 9/19/2016. <br /> On the 3/6/2017 DO report, the DO failed to document a sensor out and a fuel alarm on 2/9/2017 and 2/11/2017 <br /> (respectively). <br /> On the 7/26/2016, 9/8/2016, 10/6/2016, 11/3/2016, 12/19/2016, 1/10/2017, 2/1/2017, 3/6/2017,4/5/2017 and <br /> 5/8/2017 DO inspection reports,the DO forgot to print out the T1,T2, and T3 tank alarms. <br /> On the 9/8/2016, 10/6/2016, 11/3/2016, 12/19/2016, 1/10/2017, 2/1/2017, 3/6/2017,4/5/2017 and 5/8/2017 DO <br /> inspection reports,the DO forgot to print out the Q1 87 PLLD alarm. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 306 CCR 2630(d)Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 87 grade slave and master 323 sensors located in the submersible turbine pump sumps had fallen diagonally <br /> and were not located to detect a leak at the earliest possible opportunity. Monitoring equipment shall be maintained <br /> to be able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest point of the <br /> sump by the service technician.
