San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.slgov.org/ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Yosemite Avenue Arco AmPm 1 1711 E Yosemite Ave, Manteca June 05, 2017 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 87 master, 87 slave and 91 submersible turbine pump(STP)sumps. Liquid was also <br /> observed in the 87 slave and 91 fill sumps. The service technician on site removed a half cup of liquid from the 87 <br /> master STP sump, 1 cup of liquid from the 87 slave STP sump, a quarter cup of liquid from the 91 STP sump, 1 cup <br /> of liquid from the 87 fill sump, and trace amounts of liquid from the 91 fill sump. <br /> If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed <br /> of properly. Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> A check list was not left on site on the day of the inspection (06/05/2017). The monitoring system certification <br /> was not completed on this date. A checklist listing some of the violations were discussed with Jesse Chadda on <br /> the day of the inspection. A re-inspection must be scheduled, The owner is aware that he must pay for the <br /> re-inspection at the county rate of$139 per hour. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> Note: The spill containers, leak detectors, liquid sensors, and the fail safe tests have not been tested yet. <br /> AFTER THE COMPLIANCE DATE,EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING <br /> THIS FACILITY BACK INTO COMPLIANCE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Received by: Date: Inspector: Inspector Phone: <br /> Printed Name and Title: JOHN ALANIZ, EHS Trainee (209)468-3147 <br /> FA0003612 PR0231455 SCO01 06/05/2017 <br /> EHD 23-01 Rev.09/22/16 Page 5 of 5 Underground Storage Tank Program OIR <br />
