Meeks <br /> October 6, 2011 <br /> Health and Safety <br /> ARCADIS will modify the existing site-specific Health and Safety Plan for this scope of <br /> work as required by the Occupational Health and Safety Administration (OSHA) <br /> Standard "Hazardous Waste Operations and Emergency Response" guidelines (29 <br /> CFR 1910.120). The document will be reviewed and signed by ARCADIS personnel <br /> and subcontractors performing work at the Site. <br /> Additionally, staff performing work at the Site will have the approved "e-Rail, General <br /> Contractor Orientation, and Federal Rail Association"training certificates. ARCADIS <br /> will coordinate with UPRR to provide track protection where appropriate. <br /> Field Activities Methods and Procedures <br /> The 86 selected groundwater monitoring and remediation wells (Table 1) will be <br /> destroyed by a California C-57 licensed driller in accordance with requirements in the <br /> San Joaquin County Well Standards, dated February 2005. Supervision will be <br /> provided by ARCACIS personnel. Observations during well destruction activities will <br /> be recorded on a Well Destruction Log (Attachment 1)for each well. <br /> Well Gauging <br /> At each well location, the well depth will be gauged and compared to the existing well <br /> construction information summarized in Table 1. If debris is present in a well, it will be <br /> removed prior to well destruction activities, if possible. <br /> Well Destruction by Pressure Grouting <br /> The casing will be left in place for 56 wells (see Table 1)to be destroyed using <br /> pressure grouting technique. For these wells, the surface flush well box and/or <br /> standpipe will be removed and neat cement grout will be pumped under pressure into <br /> the well casing via a tremie pipe placed near the bottom of the well and raised as the <br /> grouting progresses. The grout will be pressurized to be forced into the formation. The <br /> grout volume placed will be calculated to confirm it exceeds the casing volume being <br /> destroyed. Concrete well vaults will be left in place if located on UPRR property but <br /> these structures will be removed from off-site properties. <br /> Following pressure grouting activities, an excavation with a diameter4arger than the <br /> well casing diameter will extend to a depth of approximately three feet below ground <br /> surface (bgs).
