In addition, the driller MUST, at least, fill the upper two portions of this LCD form and fax it to me. THe bottom of the LCD is only if the C-57 DOESN'T sign the permit application. . Clear? Most drillers who have worked in SJ county already have one of these LCD forms, check with them first. . .If needed, you can fax it to them, and they can fax it to me. . check your fax machine for the SJ County Ordinance Well Destruction requirement pages. . .the diagram puts the narrative for the pressure grout into perspective. Permits for DESTRUCTIONS are $60 per PARCEL. . .THis site, althought about 20 parcels is to tough to figure out, so one check to "SJ County EHD" for $60 will be ok. . I' ll need the work plan AND CVRWQCB approval letter, plus these forms BEFORE I can issue the permit. don't schedule a start date without checking with me first, please. .
