Yosemite, Manteca <br /> Supplemental Information Requested <br /> Gentlemen: <br /> We are in receipt of your letter dated February 22, 1999, requesting additional information for the <br /> supreme tank entry permit at the above referenced facility. <br /> The information you requested has been itemized and addressed numerically below: <br /> 1. Site EPA number is 000 043 382. <br /> 2. U.L. tank information supplied by Containment Solutions in attached letter. <br /> 3. Copy of wet stamp is attached for Engineer. <br /> 4. Copy of confined space credential for Engineer is attached. <br /> 5. Qualifications for Irene Fanelli, Industrial Hygienist, are attached. <br /> 6. Section three of application is clarified in the attached letter by the Engineer. <br /> 7. A tank test will be performed on all three (3) tanks upon completion. <br /> 8. Kent Miller of Stockton Fire Department has received a copy of the application submitted to San <br /> Joaquin County Environmental Health. We will not proceed until his needs are met. <br /> i <br /> 9. Nitrogen will be used for all tests. <br /> 10. Containment Solutions has provided an 8-hour OSHA refresher. Information is attached. <br /> 11.
