Environmental Health Department <br /> SAN 10AQUIN MENDED <br /> C_ ()IJ I'll I Y <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ARCO 06080 85 E LOUISE AVE, LATHROP January 29, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> The S11 Vacuum sensor was replaced on 1/11/2018 and a permit was not obtained from the EHD. Emergency <br /> repairs relating to UST system monitoring and release detection may be performed by a properly licensed, trained, <br /> and certified contractor pursuant to the EHUS policy and a permit obtained within one business day from the EHD. <br /> All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. Immediately <br /> obtain a permit from the EHD for the replacement of the S11 Vacuum sensor. A"Failure to Obtain a Permit' penalty <br /> applies. <br /> This is a minor violation. <br /> 304 HSC 25290.1(d) Monitoring system not maintained to detect entry into secondary containment(after Jul 2004). <br /> The dispenser#3/4 208 sensor failed to activate an audible and visual alarm when tested to ensure continuous <br /> monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm <br /> or stop the flow of product at the dispenser when it detects a leak. The sensor was replaced and retested during the <br /> inspection by the technician. A"UST Retrofit Verification with Inspector Already Onsite"form has been completed <br /> and provided to the operator and contractor. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> Weepage was observed on the diesel pipe in the dispenser#4, indicating a leak in the primary piping. All primary <br /> containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified <br /> contractor repair or replace the failed component under permit and inspection of the EHD. <br /> This is a repeat violation, Class II. <br /> 316 HSC 25290.1(c3), 25290.2(c3) Secondary containment not constructed to prevent water intrusion (after Jul 2003). <br /> Water was found in the diesel, 87, and 91 submersible turbine pump(STP) sumps, 87 and diesel fill sumps, vent <br /> box sump, and under dispenser containment(UDC) sump in dispenser#3/4. About 1/8 cups of liquid observed in <br /> the Diesel, 87, and 91 submersible turbine pump (STP)sumps, 3 cups of liquid in the 87 fill sump and 1/8 cup of <br /> liquid in the diesel fill sump, about 2 gallons in the vent box sump, and 2 cups of liquid in the under dispenser <br /> containment(UDC) sump in dispenser#3/4. Secondary containment shall be constructed to prevent any water <br /> intrusion into the system by precipitation, infiltration, or surface runoff. The service technician removed about 2 <br /> gallons of liquid from the vent box sump and returned the sensor to its proper location. Immediately contact a <br /> properly licensed, trained, and certified contractor to address the water intrusion into the diesel, 87, and 91 <br /> submersible turbine pump (STP)sumps, 87 and diesel fill sumps, vent box sump, and under dispenser containment <br /> (UDC) sump in dispenser#3/4 under permit and inspection of the EHD. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended by <br /> the EHD. The amended report replaces the initial report.
