Thornton Rd. <br /> Lodi, CA., 95242 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS-EHD) has reviewed the <br /> August— 1998 "Site Assessment Report"for the above referenced site as submitted by Advanced <br /> GeoEnvironmental (AGE) on November 12, 1998 and has the following comments. <br /> The subsurface investigation ('GP-5"through "GP-9") conducted during August 1998 that utilized GEOPROBE <br /> Technology(GP) has revealed that petroleum impacted soil exists horizontally in all directions from the former <br /> underground storage tank (UST) pit. Previous GP soil investigation ("GP-1"through "GP-4") that included <br /> groundwater samples taken in September 1997 additionally indicated that high levels of petroleum have <br /> impacted the shallow groundwater beneath the site near the UST pit. <br /> It is unfortunate that groundwater samples were not collected during this last phase of investigation. The <br /> placement of monitoring wells recommended by your consultant could have been based on the groundwater <br /> samples from the GEOPRO13E investigation. <br /> A work plan that describes the areas and number of monitoring wells to be installed is now required. <br /> Please submit the complete work plan, boring permit, and application fee to PHS-EHD by February 22, <br /> 1999. An update to the Site Specific Safety Plan should be included also. <br /> PHS-EHD requires both domestic wells (Rocky's and yours) to be sampled for the same gasoline and diesel <br /> constituents the monitoring wells will be sampled for. All groundwater samples should be collected at the same <br /> time the new monitoring wells are developed. The report of findings for this phase of work and the <br /> domestic well data should be submitted by to PHS-EHD by April 26, 1999. <br /> As noted in your State Water Resource Control Board (SWRCB) Clean Up Fund (CUF) Letter of Commitment <br /> dated December 1, 1998, you are required to comply with all regulatory agency (PHS-EHO) time schedules <br /> and requirements in order to remain eligible to receive reimbursement funds.
