(AGE) dated 09 May
2014. The San Joaquin County Environmental Health Department (SJCEHD) could not approve the work plan based on
knowledge of previously known failures in properly destroying continuous multichannel tubing(CMT)wells using
pressure grouting and over-drilling techniques.
As amended by AGE in email correspondence from you dated 1 July 2014,the nine CMT wells (MW-7 through MW-15)
are proposed to be destroyed utilizing sonic drilling. The SJCEHD approves the work plan as amended by the
aforementioned email correspondence.
Please submit an EHD Well and Boring Permit Application form along with a Master File Record form, Licensed
Contractor's Declaration form and appropriate fees. Please note that in August 2014 the EHD fee-rate may increase.
If you have any questions, please contact me at (209)468-3442 or gaeonzalezCa)s]cehd.com.
Sincerely,
Gabriel Gonzalez
From: Tim Cuellar [mailto:tcuellar(&advgeoenv.com]
Sent: Tuesday, July 01, 2014 4:16 PM
To: Gabriel Gonzalez [EH]
Subject: RE: 14749 Thornton Road Well Destruction Work Plan Addendum
We just propose to use sonic drilling on CMT wells MW-7 through MW-15.
Thanks-Tim
From: Gabriel Gonzalez [EH] [mailto:gagonzalezCobsicehd.com]
Sent: Tuesday, July 01, 2014 3:50 PM
To: Tim Cuellar
Subject: RE: 14749 Thornton Road Well Destruction Work Plan Addendum
Hello Tim,
Do you still propose to use the wire-line explosive detonation and over-drilling techniques on CMT wells MW-11
through MW-15?Or do you propose to use sonic drilling on CMT wells MW-7 through MW-15?
