HEALTH <br /> San Joaquin County Environmental Health Department PE,, ',,VVSERVICES <br /> 304 East Weber Avenue, Third Floor <br /> Stockton, California 95202 <br /> Re Well Destruction Report <br /> Shell-branded Service Station <br /> 3725 Tracy Boulevard <br /> Tracy, California <br /> SAP Code 136186 <br /> Incident No 98996222 <br /> Dear Mr Knoll <br /> Cambria Environmental Technology, Inc (Cambria) prepared this report on behalf of Equilon <br /> Enterprises LLC dba Shell Oil Products US (Shell) to document the destruction of one water <br /> supply well at the above referenced site This work was performed per Cambria's January 29, <br /> 2003 Well Destruction Work Plan The well destruction was performed to comply with San <br /> Joaquin County Environmental Health Department (SJCEHD) and California Well Standards <br /> Well Destruction Guidelines <br /> Site Background <br /> The subject site is located at 3725 Tracy Boulevard near Highway 205 in a mixed commercial <br /> and residential area of Tracy, California (Figure 1) The subject site is an operating Shell- <br /> branded service station with a station layout that includes four underground gasoline storage <br /> tanks (USTs)three dispenser islands, a car wash, and a station building (Figure 2) <br /> The water supply well was video logged on August 7, 2002 The findings of the video logging <br /> event were that the well was screened from 107 to 115 feet below grade The total depth of the <br /> well was approximately 115 feet deep The well casing was a six-inch diameter steel casing A <br /> Oakland CA concrete seal was present at the surface,but the depth of the seal was unknown <br /> San Ramon, CA <br /> Sonoma CA <br /> Monitoring Well Destruction <br /> Cambria <br /> Environmental Cambria obtained a SJCEHD well destruction permit (Appendix A) On February 14, 2003, <br /> Technology,Inc Cambria field staff observed Gregg Drilling, Inc (Gregg) of Martinez, California destroy the <br /> 46eTklns Street water supply well Gregg perforated the steel well casing using a casing ripper tool Then <br /> PO Box 259 <br /> Sonoma CA 95476 <br /> Tel(747)935-4850 <br /> Fax(707)935-6649 <br />
