-18- <br /> CHAPTER 4 <br /> CONTRACT LABORATORY QUALITY ASSURANCE <br /> QCD contracts only with laboratories which maintain the highest <br /> standards for RCRA groundwater monitoring. All analyses except <br /> for field testing (pH, conductivity) and naphthenic acids are <br /> conducted at a Department of Health Services (DOHS) certified <br /> laboratory. The contract laboratory is expected to meet the <br /> following conditions : <br /> 4 . 1 LABORATORY SAMPLE CUSTODIAN <br /> a . In the laboratory, a sample custodian receives the <br /> samples, inspects the samples and sample seals, reconciles <br /> information on the Official Sample Label and Seal against <br /> that on the Request-for-Analysis/Chain-of-Custody record, <br /> logs the sample in a laboratory log book, and stores the <br /> sample until analysis . <br /> b. The sample custodian inspects the sample container for <br /> leakage or any internal pressure which could be hazardous <br /> and cause possible loss of portions of the sample. Any <br /> discrepancies in information found on labels, seals or <br /> Chain-of-Custody records are resolved before testing . <br /> Resolution of discrepancies requires communication between <br /> laboratory supervisors, sample collectors, laboratory <br /> custodians and any others involved in the sample. All dis- <br /> cussions and observations are noted in the contract <br /> laboratory ' s receiving log. <br /> C . The laboratory custodian assigns a neutral identifica- <br /> tion number to the sample. This is the contract <br /> laboratory ' s identification number for which all work on the <br /> respective sample is filed and invoiced under . <br /> 4 . 2 QUALITY CONTROL <br /> The following are essential for the bench level quality <br /> control program: <br /> . Parameter spikes, minimum of 5% of all samples . <br /> . Matrix spikes, minimum of 5% of all samples. <br /> . Standard addition be conducted whenever significant <br /> changes in matrix background occur . <br />
