-17- <br /> Date and time of collection. <br /> Place and address of collection. <br /> Signatures of every person, in sequential order , <br /> involved in handling the sample(s) sequence of <br /> possession. <br /> Inclusive dates of possession at the refinery and <br /> analysis laboratory. <br /> 3 . 4 REQUEST-FOR-ANALYSIS FORM <br /> 3 . 4 . 1 The Request-for-Analysis form is combined with the <br /> Chain-of-Custody form (Figure 2) and accompanies the <br /> sample(s) to the laboratory. The field portion is <br /> filled out by the sample collector . It must include <br /> all required information from the Field Log . The <br /> laboratory portion is to be filled out by laboratory <br /> personnel and returned to the originator with the <br /> test results. <br /> 3 . 4 . 2 The following is the minimum information required on <br /> the Request-for-Analysis form: <br /> . Name of the person or laboratory receiving the <br /> sample (under "Sample Allocation" ) . <br /> . Laboratory sample number , collector ' s name, and <br /> company contact. <br /> . Special handling information. <br /> . Analyses to be performed, listed by parameter name <br /> and method number with reference. <br /> 3 . 5 SAMPLE OR SHIPPING CONTAINER SEALS <br /> Each sample or shipping container must have seals (Figure 3) <br /> attached in such a way that they must be broken or torn in <br /> order to remove the sample. The waterproof, gummed <br /> polypaper seal must show the following information: <br /> Date. <br /> Number of chests . <br /> Notation as to whether or not Chain-of-Custody is <br /> attached. <br />
