Project No.4-419-0381 <br /> August 29,2019 <br /> Page No.4 <br /> 1.2.6 Subcontractor Team Members <br /> The responsibilities of the Subcontractor Team Members with respect to safety are to: <br /> • Perform work safely; <br /> • Read and understand subcontractor health and safety plans; <br /> • Adhere to applicable health and safety plan protocol; <br /> • Provide applicable health and safety monitoring; <br /> • Report any unsafe acts; <br /> • Properly inspect and maintain heavy equipment and other machines in compliance with applicable <br /> sections of the federal and California Occupational Health and Safety Code; <br /> • Supply and maintain safety related protective equipment specified in this plan, such as respirators, <br /> protective coveralls,gloves, safety boots,protective eyewear, ear plugs, and hard hats; <br /> • Enforce corrective action in cooperation with the client and SALEM Project Manager; <br /> • Inform SALEM Project Manager of the presence of any potential health or safety hazards; and <br /> • Be aware and alert for signs and symptoms of potential exposure to site contaminants and climatic <br /> or acoustic stress. <br /> 2.0 HAZARD EVALUATION <br /> Certain physical, electrical, fire/explosion, chemical, climatic, and acoustical hazards are associated with <br /> the scope of work for this project. These hazards are discussed in the following sections. <br /> 2.1 Physical Hazards <br /> Field team personnel may be exposed to physical hazards around vehicles, drilling equipment, and during <br /> investigation activities. The physical hazards include: <br /> • Cuts and contusions by being struck by swinging objects, rotating objects, falling objects, and <br /> snapping cables; <br /> • Muscular injury by overexertion or improper movement(e.g.,back injury due to improper lifting); <br /> • Possible leg/ankle injury due to falling into open holes in the ground; and <br /> • Skin or eye burn during welding, cutting, or grinding activities. <br /> 2.2 Electrical Hazards <br /> Field team personnel may be exposed to electrical hazards during investigation activities. The electrical <br /> hazards include: <br /> • Puncturing buried cables; and <br /> • Exposure to overhead power lines while installing extended lengths of pipe or using equipment <br /> with extended booms. <br /> 2.3 Fire/Explosion Hazards <br /> Field team personnel may be exposed to fire/explosion hazards during all activities. The fire/explosion <br /> hazards include: <br /> • Excavating into underground pipelines carrying liquid petroleum hydrocarbons and natural gas; <br /> • Sparking of combustible organic vapors in the air,in soil gas,on soils,and on the ground water; and <br /> • Equipment fires <br /> Almost all fires are small in their early stages and can be put out quickly if the proper fire extinguisher is <br /> available and the employee has been well trained in the use of a fire extinguisher. Before beginning work <br /> at any given site you should locate and properly inspect the nearest fire extinguisher.At each new site you <br /> should make a visual inspection of the fire extinguisher.
