Project No.4-419-0381 <br /> August 29,2019 <br /> Page No.25 <br /> • When lateral or overhead clearance is close,use the assistance of someone on the ground as a guide. <br /> Always consider all lines to be live and dangerous. <br /> • If a sign warning of underground utilities is located on a site boundary, do not assume that <br /> underground utilities are located on or near the boundary or property line under the sign; have the <br /> telephone or utility company come locate the utility. The underground utilities may be a <br /> considerable distance away from the warning sign. <br /> • The Project Manager is required to contact the owners of the utility lines or the nearest underground <br /> utility locating service before drilling. The utility personnel should locate the underground lines <br /> and should mark and flag these locations. Define, with the utility personnel, what specific <br /> precautions that must be taken to perform the job safely. <br /> • Watch for sagging power lines before entering the site. SALEM employees shall never lift power <br /> lines to gain entrance. Call the utility company or client representative to request the raising or de- <br /> energizing of power lines. <br /> 4.5 Site Illumination <br /> Excavation projects sometimes require around the clock operations and therefore need temporary electrical <br /> lighting. Whenever working at night (one-half hour prior to dusk until one-half hour after dawn), sufficient <br /> lighting must be provided.The use of light towers(trailer-mounted generator/light stand units)is recommended <br /> over rig-mounted lights,as the rig-mounted lights are usually inadequate.When working at night,lights should <br /> be installed and positioned in such a way that the work area and operating positions are well lit without shadows <br /> and blind spots. In addition, all lights positioned directly above working areas should be enclosed in cages or <br /> similar enclosures to prevent loose or detached lamps or vapor tight enclosures from falling. <br /> 5.0 EMERGENCY RESPONSE <br /> Emergency response shall be implemented immediately whenever there is a fire, explosion, or release of <br /> hazardous waste or hazardous waste constituents that could threaten human health and the environment. <br /> Any field team member that sights or suspects a fire, explosion, or release of hydrocarbons, chemicals, <br /> hazardous waste, or hazardous waste constituents shall notify the appropriate authority. <br /> The SALEM field team members will use judgment in deciding the order of emergency response activities. <br /> He/she will take immediate preemptive action if safely possible, call for emergency assistance, and notify <br /> the Site Safety Officer.In the event of an emergency,the SALEM field team member will notify appropriate <br /> Emergency services and SALEM Management if an emergency occurs.
