Delineation and Destruction Work Plan for Well B-1 <br /> • Custody seals (for soil, groundwater, and IDW samples) <br /> • Sample bottles/containers (for soil, groundwater, and IDW samples) <br /> • Field data sheets and sample labels <br /> • Coolers (for sample preservation) <br /> • Ice (for sample preservation) <br /> • Decontamination supplies <br /> • Nitrile gloves <br /> • Water level indicator with oil and water detection capabilities <br /> • Drill rig and related supplies <br /> • Sealing materials (i.e., cement grout) <br /> 5 DOCUMENTATION <br /> Field notes will be maintained by Dudek personnel during the delineation and destruction <br /> activities. Photographs will be taken to document various site characteristics and field activities. <br /> The following information will be recorded in the field notebook(or on field forms): <br /> • Field observations and details related to delineation and destruction activities <br /> • Condition of the wellhead and immediate surroundings <br /> • Initial measurements of the well depth and water level <br /> • Description of removed well material and quantities <br /> • Composition of the neat cement mixture and volume used to backfill the borehole <br /> • Chronological record of work performed <br /> • Record of personnel present during this work <br /> • Summary of any meetings or discussions with contractors or regulatory agency personnel <br /> • Deviations from this delineation and destruction work plan or site-specific health and <br /> safety procedures <br /> 6 DECONTAMINATION PROCEDURES <br /> All non-disposable equipment that comes into contact with potentially contaminated soil and <br /> groundwater will be decontaminated. The equipment will be decontaminated using a steam cleaner <br /> or triple rinse, which consists of an Alconox (or similar) soap and water wash followed by a tap <br /> water rinse and a distilled water rinse. <br /> 10886-7 <br /> DUDEK 10 January 2019 <br />
