Health and Safety Plan for
Delineation and Destruction of Well B-1
2 SITE SPECIFIC SAFETY INFORMATION
2.1 Introduction
Dudek has prepared this Health and Safety Plan (HSP) for use during fieldwork activities at the
former Kearney-KPF site. A copy of this HSP will be available to all personnel working at or
visiting this site and will remain on site during all scheduled field activities.
This HSP addresses the identified hazards associated with planned field activities at the site. It
presents the minimum health and safety requirements for establishing and maintaining a safe
work environment during the course of the fieldwork. In the event of conflicting requirements,
procedures or practices that provide the highest degree of personal protection will be
implemented. If scheduled activities change or if site conditions encountered during the course of
the work are found to differ substantially from those anticipated, the Project Manager will be
informed immediately, and appropriate changes will be made to this HSP. A copy of the updated
HSP will be made available to the Safety Coordinator and all project personnel.
Any subcontractor working on the project remains responsible for identifying and evaluating
hazards (in addition to those identified by Dudek) at the site as they pertain to their activities and
for taking appropriate precautions. For example, this HSP does not address specific hazards
associated with tasks and equipment that are particular to the subcontractor's scope of work and
site activities (e.g., drill rig). Subcontractors are not to rely on this HSP to identify all hazards
that may be present at the site.
2.2 Site Description
The site is currently owned by Empire Real Estate/Alpine Builders and leased to several tenants,
most of whom operate conduct business for long-haul trucking and truck repair. There are
numerous semi-trucks onsite at all times.
2.3 Site History
Kearney-KPF was a facility that manufactured high-voltage electrical switches. Dudek conducts
field activities on behalf of the former owner, Kearney National, Inc., who is responsible for the
contamination from the former operations.
2.4 Scope of Work
The objective of this project is to decommission groundwater monitoring well B-1 at the site.
The scope of fieldwork includes the following activities:
• Mobilization/demobilization of a drilling rig, associated equipment, and personnel;
10275-4
D U D E K 3 June 2017
