Health and Safety Plan for Delineation and Destruction of Well B-1 • Delineation of impacts from the unauthorized release of an oily substance, including completion of two temporary exploratory borings for soil and groundwater sampling near well B-1; • Proper groundwater monitoring well destruction to mitigate potential threats to the groundwater, in accordance with the work plan and regulatory guidance; • Management and disposal of the investigation-derived waste (IDW); and • Documentation of site activities in a well destruction report 2.5 Responsibilities PROJECT MANAGER is responsible for the following: • Advising the Site Safety Officer on matters relating to health and safety. • Recommending appropriate safeguards and procedures. • Informing project participants of safety and health hazards identified on site. • Providing a copy of this Health and Safety Plan to field personnel prior to field activities. • Ensuring that field personnel are adequately trained. • Providing the resources necessary for maintaining a safe and healthy work environment. • Scheduling infrequent, unscheduled safety inspections of the job site. • Providing the resources necessary for maintaining a safe and healthy work environment. SAFETY COORDINATOR is responsible for the following: • Verifying that team members receive a copy of this Health and Safety Plan. • Monitoring and correcting health and safety issues noted. • Reporting workers' compensation claims. • Accident investigation and reporting. • Recordkeeping of all safety meetings and incident reports • Identifying the location and route to nearest medical facility and emergency contact information, and coordinating appropriate responses in the event of an emergency. FIELD TEAM LEAD/SITE SAFETY OFFICER is responsible for the following: • Conducting tailgate safety meetings with Dudek employees and subcontractors. 10275-4 D U D E K 4 June 2017
