Health and Safety Plan for <br /> Delineation and Destruction of Well B-1 <br /> • Delineation of impacts from the unauthorized release of an oily substance, including <br /> completion of two temporary exploratory borings for soil and groundwater sampling <br /> near well B-1; <br /> • Proper groundwater monitoring well destruction to mitigate potential threats to the <br /> groundwater, in accordance with the work plan and regulatory guidance; <br /> • Management and disposal of the investigation-derived waste (IDW); and <br /> • Documentation of site activities in a well destruction report <br /> 2.5 Responsibilities <br /> PROJECT MANAGER is responsible for the following: <br /> • Advising the Site Safety Officer on matters relating to health and safety. <br /> • Recommending appropriate safeguards and procedures. <br /> • Informing project participants of safety and health hazards identified on site. <br /> • Providing a copy of this Health and Safety Plan to field personnel prior to field activities. <br /> • Ensuring that field personnel are adequately trained. <br /> • Providing the resources necessary for maintaining a safe and healthy work environment. <br /> • Scheduling infrequent, unscheduled safety inspections of the job site. <br /> • Providing the resources necessary for maintaining a safe and healthy work environment. <br /> SAFETY COORDINATOR is responsible for the following: <br /> • Verifying that team members receive a copy of this Health and Safety Plan. <br /> • Monitoring and correcting health and safety issues noted. <br /> • Reporting workers' compensation claims. <br /> • Accident investigation and reporting. <br /> • Recordkeeping of all safety meetings and incident reports <br /> • Identifying the location and route to nearest medical facility and emergency contact <br /> information, and coordinating appropriate responses in the event of an emergency. <br /> FIELD TEAM LEAD/SITE SAFETY OFFICER is responsible for the following: <br /> • Conducting tailgate safety meetings with Dudek employees and subcontractors. <br /> 10275-4 <br /> D U D E K 4 June 2017 <br />
