Health and Safety Plan for <br /> Delineation and Destruction of Well B-1 <br /> 3 HAZARD ANALYSIS <br /> 3.1 Physical Hazards <br /> 3.1.1 Confined Spaces <br /> No confined space entry is anticipated with this scope of work. If confined space becomes <br /> necessary, entry into confined spaces will be conducted in strict accordance with 29 CFR 1910.146. <br /> Confined spaces will be evaluated prior to entry to determine if hazards are present that could pose a <br /> risk to entrants. No one will enter a confined space unless they have received confined space training <br /> per 29 CFR 1910.146, completed a pre-entry checklist, and obtained an entry permit. <br /> 3.1.2 Equipment Refueling <br /> Care shall be exercised while refueling generators, pumps, vehicles, and other equipment to <br /> prevent fire and spills. Personnel shall eliminate static electricity by grounding themselves <br /> (touching metal) prior to using refueling hoses and or containers of petroleum liquids. Items <br /> being refueled shall be grounded or be located on the ground and not on a trailer, work bench, or <br /> inside a truck bed. Emergency brake shall be used or wheels chocked to prevent accidental <br /> movement. Equipment that is hot must be allowed to cool prior to refueling. Spill response <br /> materials shall be available when conducting refueling operations. <br /> 3.1.3 Fire Protection <br /> Flammable or combustible materials must be stored in OSHA approved containers and cabinets. <br /> When working with such materials, personnel must be aware of fire and explosion hazards. <br /> Activities that may generate sparks or flames must be performed in an area away from a potential <br /> ignition source. In the event of fire, work will cease, the area will be evacuated, and the proper <br /> authorities will be notified. <br /> 3.1.4 Fleet Safety <br /> Driving is required to get to, from, and between project sites. All vehicles will be maintained <br /> properly and will be equipped with appropriate safety equipment. Drivers must have valid <br /> insurance and drivers' license with a clean driving record. Vehicle operators should exercise <br /> caution and remain alert when exiting the site, merging into traffic, and during travel. Accidents <br /> shall be reported to Facilities and Human Resources immediately. <br /> 10275-4 <br /> D U D E K 7 June 2017 <br />
