Health and Safety Plan for <br /> Delineation and Destruction of Well B-1 <br /> • Change in conditions that affect human health and safety <br /> • Change in scope of work that produce imminent hazard(s) to employees working on <br /> that task <br /> • Change in work plan that creates imminent hazard(s) that need to be mitigated before <br /> work continues <br /> To stop work, notify affected personnel and supervisors of the issue and clearly state that "we <br /> need to stop work". If necessary, remove employees from the area, stabilize the situation, and <br /> make the site as safe as possible. Discuss your concerns and observations with the supervisors <br /> and workers involved. Prior to resuming work, resolve the issue with verbal acknowledgement, <br /> change in procedure, additional PPE, etc., as needed, to maintain a safe working environment. If <br /> the issue cannot be resolved, work will be suspended until the proper resolution is achieved. <br /> Positive feedback should be given to employees regarding resolution of the stop work issue. <br /> Under no circumstances should retribution be directed by any person(s) who exercise their stop <br /> work authority in good faith. <br /> 8.4 Substance Abuse <br /> Drugs and alcohol are prohibited from any Dudek site. Any worker or oversight personnel <br /> suspected of being in an impaired condition, due to drugs or alcohol will be asked to submit to <br /> appropriate testing. Workers suspected of being in an impaired condition will not be allowed to <br /> return to work until they have confirmed their sobriety with the test results. If testing is refused <br /> or if the results are positive for drugs or alcohol, the worker will be expelled from the project. <br /> 10275-4 <br /> D U D E K 26 June 2017 <br />
