Health and Safety Plan for <br /> Delineation and Destruction of Well B-1 <br /> 9 EMERGENCY PROCEDURES <br /> 9.1 Injury or Illness <br /> All illnesses, injuries, or accidents occurring during field activities shall be attended to <br /> immediately and reported to the Site Safety Officer, Project Manager and Safety Coordinator. <br /> The injured employee shall complete the Employee Report of Injury form in Appendix B and <br /> submit to the Safety Coordinator within 24 hours of the incident. Site Safety Officer shall <br /> complete the Accident Investigation Form in Appendix B and submit to the Project Manager and <br /> Safety Coordinator. <br /> In the event of a life threatening emergency or unresponsive victim, call 911 immediately and <br /> prior to informing the Field Team Leader. If the injury is not life threatening, the injured person <br /> will be transported to the nearest hospital by a designated employee. <br /> In an emergency or hazardous situation involving explosions, fires or major physical injuries, the <br /> individual who observes this condition will immediately give a distress signal (verbal or hand <br /> gesture, depending on the circumstances). <br /> 9.2 Medical Services and First Aid <br /> Emergency phone numbers and the nearest hospital are listed in section 1 of this HSP. First aid <br /> supplies shall be readily available and stored in Dudek vehicles. First aid shall only be <br /> administered if the responder is trained and the victim is responsive and has given consent. If the <br /> victim is unresponsive, consent is implied; call 911 and administer first aid in accordance with <br /> your training. <br /> 9.3 Site Evacuation <br /> The Site Safety Officer and/or Project Manager will designate an evacuation route and refuge <br /> areas to be used in the event of a site emergency. The evacuation plan will be discussed during <br /> the daily tailgate safety meeting. Site personnel will stay upwind from dust, vapors and smoke <br /> and up gradient from spills. Personnel will assemble at the predetermined evacuation location <br /> and the Lead on site will count personnel to verify that all workers were evacuated safely. <br /> 10275-4 <br /> D U D E K 27 June 2017 <br />
