Chemical Hazards and Monitoring Procedures <br /> Chemical Hazard(s) (list): <br /> Monitoring Instrument(s): <br /> Applicable HASP Section (s): <br /> Additional Safety Considerations <br /> 1. Ensure all personnel have read the HASP <br /> 2. Ensure all equipment is equipped with necessary fire extinguishers (min 5 lbs ABC). <br /> 3. Follow safe driving procedures. Always use the buddy system when moving vehicles. Plan your travel path ahead of time. Use maps and known <br /> construction zones to make your selection. Consult with the other team members before making any changes to travel path. <br /> 4. Use an equipment checklist to verify you have the appropriate equipment/tools for your tasks. Consult appropriate JHAs or SOPs. <br /> 5. Stow all materials in vehicle properly, use appropriate cases and bags. Secure equipment in bed of truck with netting or straps. Do not leave any <br /> equipment loose in the cab or bed of the truck. It can cause property damage or serious injuries by falling from vehicle. <br /> 6. When securing equipment, watch for pinch points. Straps and netting can get caught on objects and snap back as well as trap a finger if hand placement is <br /> not correct. Use a buddy to help secure equipment when possible. <br /> 7. Maintain good housekeeping practices. When possible, use mechanical equipment to perform lifting of heavy objects.When lifting, follow safe lifting <br /> practices. Use the buddy system when lifting. <br /> 8. Wear nitrile gloves when collecting samples in soil to avoid dermal contact with potential contaminants. Be observant for tripping hazards, holes, stickups, <br /> vines, old fence wire, etc. <br /> Additional Operational Safety Procedures PPE <br /> LEVEL D <br /> • ANSI approved hard hat <br /> • ANSI approved safety glasses <br /> • Shirts with sleeves and full-length pants. <br /> • ASTM approved safety-toe boots or approved equivalent. <br /> • High visibility reflective traffic vest if near moving vehicles <br /> • Appropriate work task gloves <br /> • First aid kit(located in vehicle). <br /> • Fire extinguisher(if in EnSafe field truck). <br /> Equipment to be Used Training Requirements/Competent or Inspection Requirements <br /> Qualified Personnel name(s) <br /> 1. Various sampling media technology a. Field training on how to operate 1. Cross contamination should be avoided by <br /> equipment. practicing proper decontamination between <br /> sample sites.
