Mr.Brendan Kenney,CVRWQCB April 26,2013 Groundwater Monitoring Well Destruction Plan—Diamond Pet&Ripon Cogen Page 2 of 3 WELLS PROPOSED FOR ABANDONMENT With the exception of well OB-19,all the wells listed in Table 1 are no longer included in Diamond and Cogen's monitoring and reporting program(MRP). Wells M-7-A,M-7-B, M-11-A,M-11-B,M-11-C,M-19-C,TH-1,TH-7, TH-8,TH-9,TH-10,and TH-11, however, are included in Nestle USA's MRP No. R5-2010-0810, and it would not be appropriate to abandon them at the current time. Table 2 presents the list of wells that would be appropriate to destroy: Table 2 List of Groundwater Monitoring Wells Proposed for Destruction Zone Wells First Zone OB-3, OB-6, OB-10. OB-12, OB-13, OB-14, OB-15,OB-16,OB-18, OB-20, OB-27, Second Zone TH-2,TH-4,TH-5, Third Zone TH-3 and TH-6 FYgure 2 presents the locations of all of the monitoring wells proposed to be destroyed. PROPOSED SCOPE OF WORK CONSULTING FIRM Lawrence&Associates(L&A)will,on behalf of Diamond and Cogen,obtain San Joaquin County well-destruction permits,obtain a licensed drilling contractor to destroy the wells, supervise well destruction activities,coordinate with San Joaquin County inspectors to observe well destruction, and submit a well-destruction report to the CVRWQCB. The report will describe the methods used to abandon each well and include copies of the well abandonment permits issued by the San Joaquin County Environmental Health Department. C-57 DRILLING CONTRACTOR L&A will, on behalf of Diamond and Cogen,retain a contractor possessing a valid C-57, California well drilling contractor license to destroy the monitoring wells in accordance with all applicable State, County,and local ordinances. SAN JOAOUIN COUNTY WELL DESTRUCTION PERMITS L&A will, on behalf of Diamond and Cogen,obtain Well Destruction Permits from the San Joaquin County Environmental Health Department prior to the commencement of well-destruction activities.
