(D) The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream type listedin subdivision (c), and Its proper shipping description, including the hazardous class and United Nations/North America (UN/NA)Identification number, if applicable. (E) The name, address, and Identification number of the authorized facility to which the hazardous waste Twill be transported. (F) The transporter's name, address, and identification number. (G) The driver's signature. (H)A statement, signed by the generator, certifying that the generator has established a program to reduce the volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be economically practicable. We received a Ramos Environmental Services "Work Order Items" report that shows used antifreeze and used oil was picked up on 4-23-20. Please send copies of the disposal records. The attached paperwork (Third Attachment) is all that I received from Ramos Environmental. I will ask why they call the oil "solid" ?? I will also ask why the antifreeze is not listed anywhere. The last page is all that I received for the used oil filters. This was included in the return to compliance documents that I previously returned. 2
