What is incomplete about the Contingency Plan? I revised it and signed it, what is incomplete? I am happy to
fix it.
Or did the upload not work?
When you don't have employees, how would you like the employee training certification worked out?
Happy to comply there as well — I just don't know how to get you what you want or what certification exactly is
comprised of.
I should be able to take care of everything you want done.
I'II reach out to you on Friday the 10t" if I don't hear from you first.
Thank you.
Jason R. Mills
President and CEO
Pilot Communications
Direct: 209-949-4040
Mobile: 209-981-1010
eFax: 209.888.8980
Toll-Free: 888.745-6869
www.pilotcommunications.com
From: Robert Lopez [EH] [mailto:rlopez@sicehd.com]
Sent: October 2, 2018 11:54
To:Jason Mills<*mills@pilotcommunications.com>
Subject: PR0520262; RE: Pilot Communication Corporation Inspection Report
Mr. Mills,
I have reviewed the submittals and have found the following issues:
? The site map does not show loading areas, internal roads, emergency shutoffs, evacuation staging areas,
emergency response equipment(fire extinguishers and spill control), and the legend does not define "AC EPO"
and "DC EPO". A site map shall contain a north orientation, loading areas, internal roads, adjacent streets,
storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous
material handling and storage areas, and emergency response equipment. If a site map element is not
applicable for your facility then list it on the map and label as "NA".
Here are the guidelines for the site map:
1) All required elements need to be addressed on the map. If not applicable, write "NA" or a statement that
says the element is not on site.
2) Multiple maps can be submitted if one map cannot show all of the required elements. If more than one
map is submitted, there has to be one map that shows the overview of the entire facility.
