Santa Fe Road <br /> Riverbank,CA.,95367. <br /> By failing to submit monitoring well groundwater data in Quarterly Monitoring Reports as directed,San <br /> Joaquin County Public Health Services, Environmental Health Division(PHS-EHD) has been unable to <br /> determine if the release from your underground storage tanks (UST)has impacted the private drinking water <br /> wells located at the parcels surrounding your property. <br /> In order to determine if the private drinking wells are impacted,you are hereby directed to obtain <br /> groundwater samples from each private (domestic) water well located at each house surrounding <br /> your site. <br /> You are to obtain access to the well from the landowner/tenant prior to collecting the samples. A qualified <br /> sampling technician should be utilized for the sampling process, completion of the required Chain of <br /> Custody,and report submittal. A Department of Health Services certified laboratory is to be used for the <br /> EPA Methods, 8015m PH-G&D),8020 (B'ITN,and the Oxygenates (8260B) listed in the enclosed <br /> policy. <br /> The report of findings for the laboratory data should be submitted to PHS-EHD by September 1, <br /> 1999. <br /> Failure to comply with directives issued by PHS-EHD has resulted in your site becoming ineligible to receive <br /> reimbursement from the State Water Resources Control Board(SWRCB)Clean Up Find(CUF).
