A video <br /> survey report of DW-2 is shown on Attachment 2. The well is being destroyed as directed in a <br /> letter from the San Joaquin County Environmental Health Division (SJCEHD), dated October 9, <br /> 2000 (Attachment 3). <br /> WELL DESTRUCTION <br /> Site Specific Health and Safety Plan/Well Destruction Permit <br /> SECOR will modify the existing site-specific Health and Safety Plan for well destruction as required <br /> by the Occupational Health and Safety Administration (OSHA) Standard "Hazardous Waste <br /> Operations and Emergency Response" guidelines (29 CFR 1910.120). The document will be <br /> reviewed and signed by all SECOR personnel and subcontractors performing work at the site. <br /> A permit for the well destruction work will be obtained from the SJCEHD.
