DW-2 Well Destruction Report 2 <br /> Ranch Market 23569 South Santa Fe Road,Riverbank CA January 23,2001 <br /> supplied water to the Pantoja Property from November 16, 2000 until the replacement domestic <br /> well, DW-2R, was brought into service on January 18, 2001 <br /> Site Specific Health and Safety Plan <br /> SECOR completed the proposed work following the protocol established in the existing site- <br /> specific Health and Safety Plan, as required by the Occupational Health and Safety <br /> Administration (OSHA) Standard "Hazardous Waste Operations and Emergency Response" <br /> guidelines (29 CFR 1910 120) The document was reviewed and signed by all SECOR personnel <br /> and subcontractors prior to performing work at the site <br /> Permitting/Utility Clearance <br /> A permit for well destruction (Attachment 2) was obtained from the SJCEHD The required <br /> permitting was completed prior to performing fieldwork Prior to conducting subsurface work at <br /> the site, Underground Service AIert (USA) was contacted to delineate subsurface piping in the <br /> area surrounding the proposed work area <br /> Well Destruction <br /> On January 11, 2001, SECOR oversaw Gregg Drilling and Testing Inc (Gregg) destroy domestic <br /> well DW-2 Mr Michael Infurna with the SJCEHD was also onsite to observe the well <br /> destruction activities Gregg used a cable-operated retractable blade to perforate the 61/4-inch <br /> diameter steel casing The casing was perforated with 3-inch vertical cuts every 18-inches from <br /> depth to five feet below ground surface (bgs) and the tool was rotated 90 degrees between cuts <br /> (Field Notes, Attachment 3) After the casing was perforated, Gregg grouted the well from total <br /> depth (60 feet bgs) to five feet bgs <br /> Gregg removed the well's concrete pad and excavated to three feet bgs with a backhoe, then cut <br /> the 61/4-inch diameter steel casing off at approximately 21/2 feet bgs Gregg then placed an 18- <br /> inch tall modified 55-gallon drum in the hole to surround the exposed six inches of casing <br /> Gregg continued grouting, filling the rest of the steel casing and the drum, creating a grout <br /> mushroom cap on top of the casing Gregg backfilled the excavation with soil and compacted it <br /> by wheel rolling with the backhoe <br /> File DW 2Dc5uWi doc <br />
