nd <br /> San Joaquin County Human Services May 4, 2015 <br /> Work Plan Approval Page 2 of 2 <br /> 145 S Sutter Street <br /> After issuance of the permit, the EHD requires inspections to be scheduled with Johnny <br /> Yoakum, REHS at least 5 workdays prior to implementing fieldwork. Mr. Yoakum may be <br /> contacted via email at JYoakum@sjcehd.com or(209) 468-3147. <br /> All investigation-derived wastes, including soil cuttings, debris, groundwater and rinsate <br /> generated from soil borings and decontamination procedures, shall be containerized for proper <br /> disposal and the containers labeled to characterize the waste: hazards, composition, generation <br /> site address, generator's name, and date of accumulation. <br /> The site has not had a groundwater monitoring event since 2011; current data is needed to <br /> evaluate the site for case closure. You are directed to schedule and conduct a groundwater <br /> monitoring event that includes all monitoring wells associated with this investigation by 30 June <br /> 2015 and submit a report of findings no later than 31 August 2015. In addition to the suite of <br /> analytes previously analyzed, please include naphthalene for the monitoring well MW-2 for this <br /> event. <br /> There appears to be a gap in the groundwater monitoring record between 2008 and 2011 <br /> followed by four monitoring events in 2011 and one in 2012, but no reports on these monitoring <br /> events have been submitted to GeoTracker, the database maintained by the California State <br /> Water Resources Control Board for investigations and cleanups of sites impacted by <br /> unauthorized releases from leaking underground storage tanks. Submittal of such reports to <br /> GeoTracker is required under Title 23, Division 3, Chapter 30, Articles 1 and 2, Sections 3890, <br /> 3891, 3892, 3893, 3894 and 3895 of the California Code of Regulations.
