1 <br /> Advanced <br /> GeoEnvironmental, Inc , <br /> 13 January 2010 <br /> AGE-NC Project No. 97-0343 <br /> Ms. Lori Duncan 1 9 <br /> San Joaquin County Environmental Health Department SA �EALjH <br /> 600 East Main Street ENVIRON S�RVIGES <br /> Stockton, California 95202 <br /> Subject: Well Destruction Report <br /> Former FISCO WAREHOUSE (Howard Lenz Property) <br /> 1648 Shaw Road, Stockton, California <br /> Ms. Duncan: <br /> At the request of Mr.Peter Lenz on behalf of Mrs.Alice G.Lenz,Advanced GeoEnvironmeratal,Inc. <br /> (AGE)has prepared this report for the destruction of three dual-completed ground water monitoring- <br /> soil vapor extraction wells (MW-INW-1, MW-2NW-2, and MW-3NW-3), and nine single- <br /> completed ground water monitoring wells (MW-4 through MW-12) at 1648 Shaw Road, Stockton, <br /> California(site).The site location is depicted on Figure I;site structures and former monitoring well <br /> locations are depicted on Figure 2. <br /> INTRODUCTIONBACKGROUND <br /> Three dual-completed two-inch diameter ground water monitoring-soil vapor extraction wells <br /> (borehole diameter 10 inches)and nine single-completed two-inch diameter ground water monitoring <br /> wells had been installed as part ofa subsurface investigation of the former underground storage tank <br /> (UST)area at the site.Dual-completed ground water monitoring and soil vapor extraction wells were <br /> installed to approximate depths of 28 feet and 55 feet below surface grade (bsg); monitoring wells <br /> MW-4 through MW-7 were installed to depths of 60 feet bsg; ground water monitoring well MW-8 <br /> was installed to a depth of 95 feet bsg; and ground water monitoring wells MW-9 through MW-12 <br /> were installed to depths of 70 feet bsg. <br /> A San Joaquin County Environmental Health Department (EHD)-prepared letter dated 27 August <br /> 2009 stipulated monitoring and vapor extraction wells be destroyed before final closure at the site <br /> would be granted.
