%.I <br /> 13 January 2010 <br /> AGE-NC Project No. 97-0343 <br /> Page 2 of 2 <br /> PROCEDURES <br /> Well destruction activities were performed on 16, 17, and 18 November 2009 by All Well <br /> Abandonment of Placerville, California. Dual-completed ground water monitoring and soil vapor <br /> extraction wells (MW-1 /VW-1, MW-2 /VW-2 and MW-3 /VW-3) were destroyed by pressure <br /> grouting the well columns with Portland Type II cement(six gallons water for every 94-pound sack <br /> of portland cement)and over-drilling the upper three feet of the well using 10-inch diameter hollow <br /> stem augers.Ground water monitoring well MW-9 was removed by over-drilling the well to its total <br /> depth (70 feet bsg)with 8-inch diameter hollow stem augers and a solid drive lead-bit; well MW-9 <br /> boring was then grouted to near surface using a portland cement grout mixture. Remaining ground <br /> water monitoring wells (MW-4 through MW-8 and MW-10 through MW-12) were destroyed by <br /> pressure grouting the well columns with Portland Type II cement and over-drilling the upper three <br /> feet of the well using 8-inch diameter hollow stem augers. The upper surface was backfilled(grout <br /> or clean soil) and covered with concrete or road base. <br /> On 08 December 2009,55-gallons of non-hazardous rinseate(generated from monitoring events and <br /> well destructions) and 3,600 pounds of soil cuttings and solid waste (generated from well <br /> destructions)containerized in 55-gallon drums were removed from the site by Advanced Chemical <br /> Transport, Inc. (ACT) of Merced, California. Non-hazardous waste was disposed at US Ecology <br /> Nevada, Inc. located near Beatty, Nevada.
