The following is an itemized list of hazardous waste violations that have not been addressed for Zacky Farms as of April 03, 2020. Open violations from September 25, 2019 inspection floor. Employee Break Room F2: -Quaternary ammonia compounds were used to clean the floors. A white crystal/powder substance covered the floor. Locker Room F2: -A white crystal/powder substance covered the floor. -Five spray bottles containing an unknown liquid were observed in one of the lockers. -According to Mr. Mancha,the employees were unexpectedly"let go." Many of the lockers had locks on them and were not inspected. A hazardous waste determination needs to be made for any contents inside the rest of the locker. Maintenance Room (F2): -Six lead acid batteries were observed on a shelf. Main Hallway Storage Room (F2): -A flammable cabinet contained: Two 5 gallon buckets labeled Gear Lubricant(full). Two boxes containing several tubes labeled "DAP"(full). Four 1 gallon paint cans(full). Laboratory(F2): -Two Peracetic Acid Test Reagent kits(located in cabinets). -Four 500 milliliter bottles(full)labeled Sodium Thiosulfate, 0.1 N (located inn cabinets). -Multiple ChemStation chemicals on the counter. One 1 L bottle(full)was labeled"Sodium Hydroxide, 1.ON. -One 5 gallon bottle(full)labeled Sodium Hydroxide Solution 1 N. -One 1 gallon bottle (1/2 full)labeled Sulfuric Acid was observed in the fume hood. -Beneath the fume hood there was a storage area labeled "Chemical Waste from Lab Experiments." Inside the storage area were three 1 gallon plastic containers, one 5 gallon plastic container, and two smaller plastic bottles (less than'h a pint). None of the labels on the containers were able to be read. A pile of fine white powder was located on a tray next to the chemicals. -One the counter there was one 250 milliliter flask(1/2 full)containing a white fluid. The flask was labeled"Bleach." Next to the bleach flask were four 400 milliliter bottles. Two of them were labeled"Buffer Solution pH 7.0." -Next to the flask noted above was another 250 milliliter flask containing a dark liquid. The flask did not have a contents label. Attic Area West Side(F3): -Two 1 gallon plastic buckets contained an unknown material. -Two 1 gallon plastic bottles labeled"Orange Power." -One 5 gallon box labeled"Nandi Foam." Attic Area Central (F3): -One 5 gallon box labeled "Nandi Foam." -One open blue 55 gallon poly drum contained an unknown clear liquid with an orange sediment at the bottom. -One open blue 55 gallon poly drum contained an orange sludge floating on top of a dark liquid. -One 5 gallon box labeled"Nandi Foam"was placed next to a one gallon bottle labeled "White distilled Vinegar." The one gallon bottle(1/4 full) held an orange liquid. A dark heavy staining was observed on the floor below the items. Attic Area East Side(F3): -One 3 gallon bucket labeled"Greenlee Poly Line." -One box contained approximately seven 1 gallon containers labeled"Kill Quick Mosquito Fog with Pyrethroid." Next to the containers was another 1 gallon container holding an unknown material. Page 4 of 14
