8.3 Heat Stress <br /> Heat stress monitoring shall be initiated whenever ambient temperatures on site exceed 85°F <br /> At the discretion of the Project HS Manager or FC, environmental and/or physiologic <br /> monitoring will be carred out <br /> 8.4 Safety Review <br />' At least once during the project, both the PM and a health and safety professional will carry <br /> out a comprehensive safety review of the project. The FC or project manager will conduct <br /> site safety inspections at least weekly, and take effective corrective action. These weekly <br /> inspections will be documented on FADL forms. Management safety reviews will be <br />' recorded on Safety Inspection Report Forms and will be forwarded to the HS Department for <br /> review <br />' 8.5 Monitoring Records <br /> The PM shall ensure that site monitonng records are complete and incorporated into the <br />' project file Any personnel or area air monitoring results will be incorporated into the host <br /> office health and safety files. The host office HS Manager will be responsible for <br /> establishing, maintaining, and forwarding to other rr offices (as necessary) all required <br /> monitoring information as described below for placement in individual employee files: <br /> • Employee name, social security number, payroll number <br /> • The date, time, pertinent task information, exposure information <br />' Description of the analytical methods, equipment used, and calibration data <br /> • Type of PPE wom <br /> • Engineering controls used to reduce exposure. <br /> 8.6 Notification <br />' The project health and safety staff and the host office health and safety staff will ensure that <br /> each employee is informed in writing of the results which represent that employee's exposure. <br /> Monitoring results representative of an employee's exposure shall be reported in writing to <br />' the affected employee. <br />' Whenever the results indicate that the representative employee exposure exceeds the <br /> Permissible Exposure Limit (PEL), the notification shall state that the PEL was exceeded, and <br /> M710354o¢94/UST/94-0063 bk 8-3 <br />
