' 10.0 Medical Surveillance Program <br /> 10.1 Physical Examinations <br /> All on-site project personnel shall within the past 12 months have completed a comprehensive <br /> medical examination that meets the requirements of OSHA's Regulation 29 CFR 1910120 <br /> The annual medical includes the following elements <br /> Medical and occupational history questionnaire <br /> • Physical examination <br /> • Complete blood count, with differential <br /> Liver enzyme profile <br />' Chest X-ray, once every three years, for non-asbestos workers <br /> • Pulmonary function test <br /> • Audiogram <br />' Electrocardiogram (EKG) for persons older than 35 years of age, or of indicated <br /> during the physical examination <br /> • Drug screening <br /> - Visual acuity <br /> • Follow-up examinations, at the discretion of the examining physician or the <br /> corporate medical director <br /> All employee medical records are maintained by the health and safety group, within the <br /> worker's home profit center, or for subcontractors at the subcontractor's office The <br /> examining physician provides the employee with a letter summarizing his findings and <br /> recommendations Each employee also has the right to inspect and copy medical records. <br />' The examining physician provides the employer with a letter confirming the worker's fitness <br /> for work and ability to wear a respirator. A copy of this letter for all subcontractors will be <br /> kept on site during all project site work. <br /> Subcontractors will certify that all their employees have successfully completed a physical <br />' examination by a qualified physician on the Certification Form.
